{"id":12302,"date":"2026-03-04T11:10:21","date_gmt":"2026-03-04T11:10:21","guid":{"rendered":"https:\/\/magazines.uthscsa.edu\/mission\/?p=12302"},"modified":"2026-03-30T18:04:59","modified_gmt":"2026-03-30T18:04:59","slug":"a-sound-solution-to-tremors","status":"publish","type":"post","link":"https:\/\/magazines.uthscsa.edu\/mission\/a-sound-solution-to-tremors\/","title":{"rendered":"A sound solution to tremors: Focused ultrasound offers relief without surgery"},"content":{"rendered":"<h4><span style=\"color: #000000;\"><em>High-frequency, focused ultrasound treatment gives a South Texas couple the stillness they\u2019ve been seeking<\/em><\/span><\/h4>\n<p>&nbsp;<\/p>\n<p>For more than five decades, Deeann Hall learned to live with the vibration in her hands. It\u2019s something that began as a faint tremor in childhood, but by her late fifties, became an exhausting daily challenge.<\/p>\n<p>Drinking coffee meant using both hands and still spilling. Forkfuls of food fell away before reaching her mouth. Even signing her name or brushing her teeth felt like monumental battles against her own body.<\/p>\n<p>Her husband and high school sweetheart, Rick Hall, watched as her frustration deepened year after year.<\/p>\n<p>\u201cIt was getting to where I was going to have to start helping her with everything,\u201d he recalled. \u201cSeeing her struggle \u2014 it broke my heart.\u201d<\/p>\n<p>The culprit was a condition known as <a href=\"https:\/\/lsom.uthscsa.edu\/neurosurgery\/clinical-practice\/treatments\/essential-tremor\/\">essential tremor<\/a>, a neurological disorder that affects millions of Americans and is sometimes passed down in families. Unlike Parkinson\u2019s disease, essential tremor doesn\u2019t cause stiffness or slowed movements but leads to involuntary shaking, particularly during activity, and often worsens over time.<\/p>\n<p>For Deeann Hall, whose condition ran through her father\u2019s side of the family, the tremors had become a constant, unwelcome companion.<\/p>\n<p><iframe src=\"\/\/www.youtube.com\/embed\/hbaL7cZKXn8\" width=\"844\" height=\"473\" allowfullscreen=\"allowfullscreen\"><\/iframe><\/p>\n<h4><span style=\"color: #333333;\"><strong>Searching for answers <\/strong><\/span><\/h4>\n<p>A few years ago, the couple began looking for options beyond medications because Deeann Hall wasn\u2019t seeing any improvement. One evening, while scrolling online, she came across information about a new therapy called <a href=\"https:\/\/uthscsa.edu\/physicians\/services\/focused-ultrasound\">focused ultrasound<\/a>, described as an incisionless brain procedure that could reduce tremors without surgery.<\/p>\n<p>\u201cNonsurgical is the word that caught my attention,\u201d she said. \u201cIt claimed they wouldn\u2019t have to cut anything.\u201d The Halls were pleasantly surprised when their neurologist informed them that UT Health San Antonio would soon launch the region\u2019s first focused ultrasound program under the direction of <a href=\"https:\/\/cancer.uthscsa.edu\/doctors-and-locations\/doctors-and-clinics\/alexander-papanastassiou\">Alex Papanastassiou, MD<\/a>, a neurosurgeon and director of stereotactic, functional and epilepsy neurosurgery.<\/p>\n<p>Within a few months of first meeting Papanastassiou, Deeann Hall became the second patient the program scheduled after its launch.<\/p>\n<p>\u201cDeeann had essential tremor affecting both sides of her body,\u201d Papanastassiou said. \u201cBecause she\u2019s righthand dominant, we knew we needed to first treat the left side of her brain, which controls the right hand. The tremor made everyday tasks nearly impossible \u2014 writing, eating, even carrying a cup or a plate.\u201d<\/p>\n<p>In preparation for the procedure, Deeann Hall was informed she would have her hair shaved to prevent air bubbles from interfering with the ultrasound treatment. The night before surgery, her husband showed his support in an unforgettable way by shaving his own head.<\/p>\n<p>\u201cShe told me I didn\u2019t have to, but I told her I did. We\u2019re in this together,\u201d he said.<\/p>\n<h4><span style=\"color: #333333;\"><strong>A procedure of precision <\/strong><\/span><\/h4>\n<p>On Aug. 5, the team began before sunrise, shaving Deeann Hall\u2019s head and fitting her with a head frame to keep her still during the MRI-guided procedure.<\/p>\n<p>\u201cPatients stay awake and aware during the procedure,\u201d Papanastassiou said. \u201cThis is so we can test for tremor reduction, side effects and adjust the target if needed before making a permanent lesion.\u201d<\/p>\n<p>Once on the MRI table, cold water circulated through a specialized cap secured to the head frame to protect Hall\u2019s scalp and allow the ultrasound waves to travel from the transducers to her brain.<\/p>\n<p>Papanastassiou aimed for a tiny target in the thalamus called the ventral intermediate nucleus, or VIM, for delivery of focused high-frequency sound waves. Such procedures exemplify the advanced, precision-based care now available in South Texas at UT Health San Antonio.<\/figcaption><\/figure>\n<p>When Rick Hall walked into recovery, he saw his wife crying, but for the best possible reason.<\/p>\n<p>\u201cShe held up her hands and said, \u2018Look!\u2019\u201d he recalled. \u201cLater, she picked up a cup of coffee without a lid, took a drink and set it back down. No shaking. It was like watching a miracle.\u201d<\/p>\n<p>The tremor in her right hand had quieted \u2014 95% gone within minutes. Even her head and voice tremors, which improve in about half of patients, disappeared.<\/p>\n<p>\u201cBefore treatment, she couldn\u2019t even draw a spiral; it was just scribbles,\u201d Papanastassiou said. \u201cBy the end, she was tracing it smoothly with almost no tremor. To watch her reaction in real time, right before our eyes, was incredibly rewarding.\u201d<\/p>\n<p>By lunchtime, Deeann Hall was eating with ease. That evening, she felt something far greater: freedom.<\/p>\n<h4><span style=\"color: #333333;\"><strong>Life after tremors <\/strong><\/span><\/h4>\n<p>According to Papanastassiou, unsteady walking may occur in about half of patients and typically resolves in two to four weeks, as it did for Deeann Hall.<\/p>\n<p>\u201cThey told me I might be a little wobbly afterward,\u201d Deeann Hall said, laughing. \u201cI didn\u2019t realize how true that was. I was so used to compensating for the tremor that when it was gone, I\u2019d bump into walls because my body wasn\u2019t used to being steady.\u201d<\/p>\n<p>In May, Deanne Hall will return for her second treatment to address the right side of her brain. Until then, she\u2019s savoring all the activities she can now do.<\/p>\n<p>\u201cI can trim my plants again without breaking them,\u201d she said. \u201cI can drink my coffee, write my name and just do normal things.\u201d<\/p>\n<p>For her husband, the joy was doubled.<\/p>\n<figure id=\"attachment_12530\" aria-describedby=\"caption-attachment-12530\" style=\"width: 263px\" class=\"wp-caption alignleft\"><img decoding=\"async\" class=\"wp-image-12530 \" src=\"https:\/\/magazines.uthscsa.edu\/mission\/wp-content\/uploads\/sites\/20\/2026\/03\/FUS_mrs_hall-215x300.jpg\" alt=\"\" width=\"263\" height=\"367\" srcset=\"https:\/\/magazines.uthscsa.edu\/mission\/wp-content\/uploads\/sites\/20\/2026\/03\/FUS_mrs_hall-215x300.jpg 215w, https:\/\/magazines.uthscsa.edu\/mission\/wp-content\/uploads\/sites\/20\/2026\/03\/FUS_mrs_hall-107x150.jpg 107w, https:\/\/magazines.uthscsa.edu\/mission\/wp-content\/uploads\/sites\/20\/2026\/03\/FUS_mrs_hall-350x489.jpg 350w, https:\/\/magazines.uthscsa.edu\/mission\/wp-content\/uploads\/sites\/20\/2026\/03\/FUS_mrs_hall.jpg 688w\" sizes=\"(max-width: 263px) 100vw, 263px\" \/><figcaption id=\"caption-attachment-12530\" class=\"wp-caption-text\">After focused ultrasound treatment, Deeann Hall can now fully enjoy the activities she loves, including caring for her thriving collection of plants.<\/figcaption><\/figure>\n<p>\u201cShe\u2019s happy,\u201d he said. \u201cSeeing her do what she loves \u2014 it just brings so much joy to me.\u201d Their family was equally moved.<\/p>\n<p>\u201cOur children cried,\u201d Deeann Hall said. \u201cThey\u2019d never known me without the shaking.\u201d<\/p>\n<h4><span style=\"color: #333333;\"><strong>\u2018A modern miracle\u2019 <\/strong><\/span><\/h4>\n<p>Today, the Halls call the focused ultrasound procedure \u201ca modern miracle,\u201d and they have become unofficial ambassadors for the treatment, telling everyone they meet who might benefit.<\/p>\n<p>\u201cThis kind of success is only possible because of the multidisciplinary team here at UT Health San Antonio\u2019s Multispecialty and Research Hospital,\u201d Papanastassiou said.<\/p>\n<p>\u201cEveryone on our team, including nursing, neurology, radiology, anesthesia, ENT, PMR, physical and speech therapy as well as our support staff, all play a role in helping patients feel safe and cared for throughout the process,\u201d he added.<\/p>\n<p>\u201cWhen you see a patient\u2019s hand become still after years of tremor, you know you\u2019ve changed a life.
